What is medication therapy management (MTM)?
MTM is a one-on-one consultation that we schedule to review all the medications, you are taking to make sure they are appropriate and effective.
In this consultation, you can become actively involved in your medications. Any questions you may have about your medications, including non-prescription medications, vitamins, and herbal remedies, will be answered by a pharmacist. Review of your vitamins, supplements and over-the-counter drugs will be reviewed to make sure they are safe, effective and necessary. Side effects will be reviewed as well as how to manage those side effects. Having a pharmacist review all of your medications allows them to make sure you’re only taking the medications you need, that way, you can save money and have fewer side effects.
During the meeting, the pharmacist also makes sure you are taking the correct amount of medicine at the right time and that your medications do not interact with any other prescription or over-the-counter medications you are taking. Anything that is spoken during the consultation is relayed to your doctor by fax and mail. By working with your physician, the pharmacist can make sure everything is done to meet your goals of therapy and to ultimately improve your health.
A pharmacist can also teach you how to use any medical supplies you might need to use, such as blood sugar testing and blood pressure testing.
Patients who may benefit the most include:
- Patients who have just been discharged from the hospital
- Have added/removed medications from their current regimen
- Have multiple diseases or take multiple medications
- Have a history of forgetting to take the medications
- Want to reduce cost of medications
Medication Therapy Management is available to anyone who is concerned about their own, or a loved ones, medication usage. These meetings may take 30 minutes or longer and are held in a private area of our New York and Brooklyn offices. You are welcome to have a caregiver, or someone who is a part of your medication routine accompany you. After the meeting, the pharmacist will provide you with a Medication List, Action Plan and educational materials based on your discussion.
It is a free service, completely covered under Medicare and some state Medicaid programs. Call to schedule a private appointment with one of our New York pharmacists today!
