What is medication therapy management (MTM)?

MTM is a one-on-one consultation that allows us to review all of the medications you are taking in order to make sure they are appropriate and effective.

In this consultation, you can become actively involved in your medications. Any questions you may have about your medications, including non-prescription medications, vitamins, and herbal remedies, will be answered by a pharmacist. Vitamins, supplements and over-the-counter drugs will be reviewed to make sure they are safe, effective and necessary. Side effects will also be evaluated, and we will also discuss how to manage those side effects. Having a pharmacist review all of your medications allows him or her to make sure you’re only taking the medications you need, which can reduce your side effects and save you money.

During your meeting, the pharmacist also makes sure you are taking the correct dosage and at the right time, and that your medications do not interact with any other prescription or over-the-counter medications you are taking. Anything that is discussed during the consultation is relayed to your doctor by fax and mail. By working with your physician, the pharmacist can make sure your therapeutic goals are met, and ultimately improve your health.

One of our pharmacists can also teach you how to use any medical supplies you might need to use, such as blood sugar testing and blood pressure testing.

  • Patients who have just been discharged from the hospital
  • Those who have added/removed medications from their current regimen
  • People with multiple diseases or who are taking multiple medications
  • Those who have a history of forgetting to take the medications
  • Anyone who wants to reduce cost of medication

Medication Therapy Management is available to anyone who is concerned about their own medication usage, or the medication usage of a loved one. Meetings may take 30 minutes or longer and are held in a private area of our New York and Brooklyn offices. You are welcome to have a caregiver, or someone who is a part of your medication routine accompany you. After the meeting, the pharmacist will provide you with a medication list, an action plan and educational materials based on your discussion.
